KIndergarten Registration

Complete the form below to have a registration packet mailed to you.

Cambridge Springs Elementary School is gathering information to determine the number of children in our attendance area who will be old enough to attend Kindergarten during the school year. To enroll in kindergarten, the following requirements must be met:

All children must be:

• 5 years of age on or before June 30 to register. (a copy of the birth certificate or other proof of age MUST be provided on registration day.)

All students entering Kindergarten must have the following:

• 4 DTaP (Diphtheria, Tetanus) with the fourth dose after age 4
• 4 Polio with the fourth dose after age 4
• 2 Measles after age 1
• 2 Mumps after age 1
• 1 Rubella after age 1
• 3 doses of Hepatitis B
• 2 doses of Varicella (chickenpox) immunization or history of disease.

Proof of immunizations must be provided at registration.

If you know anyone with a child who will be entering kindergarten in the fall, please share this information with them. Parents may also contact the elementary office at (814)398-4636.

Registration forms and other important information regarding kindergarten registration will be mailed to parents who have completed and submitted this form or who have contacted the Elementary Office.
